Absentee Ballot Request Form


Any registered and qualified voter may vote an absentee ballot.  In addition, all members of the Armed Forces, their family members and members of the Merchant Marine and their family members, who are U.S. citizens, may vote absentee.  For more information on overseas absentee voters (UOCAVA), please visit the Federal Voting Assistance Program web site at www.fvap.gov.

Call (850-983-1900) or e-mail (soe-bodenstein) our office to ask about absentee balloting as soon as you know that you will be unable to go to the polls on Election Day.  When requesting an absentee ballot, you must provide your name, date of birth, and residence address or address which the ballot should be mailed. 

One request shall be deemed sufficient to receive an absentee for all elections through the next two general elections after the request is made, rather than just for the elections in the calendar year in which the request was made.  Or you can request just one election if that is all you need. You may make your request in writing (6495 Caroline St;, Suite “F”, Milton Fl 32570-4592), by telephone (850-983-1900), e-mail (soe-bodenstein), or fax (850-626-7688).  Requests may also be made by the voter’s immediate family or legal guardian if instructed by the voter.

NOTE: Absentees for the Primary Election, August 26, 2008, can not be accepted.  The deadline was Wednesday, August 20 at 4:30.  Any request after this date will be for the General Election in November.
