We/I the homeowner(s) of Quiet Street request that a Municipal Service Benefit Unit (MSBU) be established for the purpose of installing and maintaining street lighting on Quiet street in the Holley by the Sea development. We would like to have street lights installed on Quiet Street starting from Bird Whistle street to Duck Bill Street (or list the name of the subdivision).

I have enclosed the written estimate for the street lights that was provided to me by Mr./Ms. _______________ Gulf Power representative.

I will be the point of contact for all correspondence and will ensure the petitions are circulated.
